Carehome Selection provides a major placement service for hospitals and PCTs across north and west London.

Known as the North Middlesex Pilot, the service involves CHS taking referrals for patients in hospitals within Barnet, Enfield, Harringey and Hillingdon, Middlesex and Ealing.

Carehome Selection advisers meet hospital patients and their families, identify suitable care homes and support them with visits and all paperwork, as required.

The service reduces delayed discharges and enables patients to move from hospital into a more suitable environment without unnecessary delays.

The scheme is completely free to the NHS and to the patient. Carehome Selection is paid a percentage of carehome fees to cover the costs of this work. The percentage is fixed to ensure Caretrack is independent and does not favour one provider over another.
