Carehome Selection's placement scheme for Liverpool PCT

Carehome Selection has started a placement scheme for Liverpool Primary Care Trust supporting hundreds of continuing health care patients.
The work follows a successful 'data cleanse' undertaken by Carehome Selection to check and verify all the information held on individuals receiving continuing healthcare. In Liverpool, this involved 1,500 clients.
Carehome Selection delivered the project and was commissioned by Liverpool PCT to undertake further work, assisting patients and their families to find suitable care homes.
Local staff were recruited, led by a skilled and experienced manager from Carehome Selection, to assist patients who have been awarded continuing health care and their families.
Carehome Selection liaises with the family, care homes and all health and social care professionals, to ensure the placement meets the client's needs and there are no unnecessary delays.
Placement schemes are run by Carehome Selection across the country, ensuring patients receive appropriate care and hospital beds are available for those in need of acute care.
Liverpool PCT initially commissioned Carehome Selection to 'data cleanse' all their information about continuing health care patients.
In common with many PCTs, this information was held in a variety places using different systems. Once the information was checked and verified, Carehome Selection amalgamated all the data onto a purpose-designed system called Caretrack created for continuing healthcare information.
ate Craddock, Commissioning Lead for Older People's Services, Liverpool Primary Care Trust explains why Carehome Selection was chosen to work for the PCT: "We wanted to bring together all the information which was stored in different parts of the PCT in relation to the funding of people with complex needs.
"We recognised that we needed to amalgamate all this information in order to be able to predict demand for services in the future.
"We went to Carehome Selection basis of a recommendation from another PCT.
"The attraction of Carehome Selection was in knowing the company already work in the continuing healthcare sector and understand the relevant policies and initiatives.
"It was not just a case of an IT company asking us what we wanted from a new database.
"It has been very much a partnership - Carehome Selection has advised us on all the different fields and functions we need on the database in order to meet our objectives."
