Role Summary

Our Trusted Assessors work to the highest standards with a commitment to ensure that some of society’s most vulnerable people transition efficiently at the point of hospital discharge. With the support of the wider CHS team coupled with your sound clinical and business knowledge, you will complete robust assessments for patients leaving hospital ensuring appropriate packages of care meet their needs in both domiciliary and care home settings. 

You will work with care providers and hospital discharge teams to develop jointly agreed protocols and procedures for discharge assessment and decision-making. Working collaboratively with care homes, you will gain a comprehensive knowledge of their dynamics and structure, identifying their changes and updates ensuring effective and thorough assessments are delivered on their behalf.

You will support care homes with a high percentage rate of re-admissions into Hospital, providing root cause analysis on why, whilst identifying solutions. 

This is a complex role for which you will provide high quality support including complex information and analysis, communications, and stakeholder management. You will be primarily based at James Cook University Hospital but will also be required to visit Residential and Nursing Homes on a regular basis working in partnership to ensure services are delivered efficiently and effectively.  You will also be required to provide cover for absences at other sites in the North East.
