You will be required to work flexibly across a 7-day service – 40 hours per week

This role will particularly suit a RGN with experience in the Care Home with Nursing Environment

We will need you to undergo an enhanced check with the Disclosure and Barring Service.

About the role

Are you a highly motivated and enthusiastic professional looking for a new challenge or currently working within the Care home with Nursing environment?

Working to the highest standards, you will be responsible for completing robust assessments for patients leaving hospital to ensure they have an appropriate package of care to meet their needs in either a domiciliary setting or within a care home setting.

Following a period of in-patient care, and prior to an individual’s discharge, the role of the Trusted Assessor is to undertake an independent and unbiased assessment to determine suitability and requirements on behalf of care providers.

This is a complex role completing and co-ordinating assessments of the client needs ready for discharge from hospital. You will provide high quality support including complex information and analysis, communications, and stakeholder management. You will be primarily based at University of North Durham hospital but will also be required to visit Residential and Nursing Homes on a regular basis working in partnership to ensure services are delivered efficiently and effectively.

You will also be required to:

  • Establish and maintain effective and trusted collaboration with care providers to ensure that effective and thorough assessments are delivered on behalf of care providers
  • Based on assessment, make recommendations to care providers discharge decisions
  • Work with care providers and hospital discharge teams to develop jointly agreed protocols and procedures for discharge assessment and decision-making
  • Working with the current CHS team ensuring smooth transition from hospital to Care Home
  • Work with care homes to gain comprehensive knowledge of the dynamics and structure of the home and ensure regular updates are obtained to identify any changes made within the home
  • Working with homes that have a higher percentage rate of re-admissions into Hospital and identifying the reasons why, if there are any patterns to this and provide possible solutions
  • Provide cover for annual leave/absence at our other schemes in the local area

You will require access and use of your own vehicle with car insurance that covers you for business use.

What sort of person are we looking for?

You will be a self-motivated, driven professional who has the ability to manage your own caseload to complete within an agreed KPI timescale.

Ideally degree educated or with relevant professional qualification. You must be a registered nursing professional who is outgoing, friendly with a desire to build supportive long-term relationships with clients, colleagues and a wider professional network with an essential ability to “run your own show”.

You must pride yourself on your writing, presenting and selling skills as you will need to persuade others and move them into action. You will also need strong IT, administration and analytical skills.
