What is the Carehome Placement Service?

This is a service first developed by Carehome Selection in 1995. Patients who are ready for discharge are given the option of support from a Carehome Selection adviser, who will work with the client and their family to identify suitable homes and assist with the paperwork and all arrangements.

A Carehome Selection Hospital Co-ordinator manages this work, providing the hospital with regular updates about all clients.

This service has been proven to significantly reduce the number of delayed discharges.
