What is an Admission Avoidance Scheme?
Studies show a significant proportion of patients who are admitted to hospital via A&E do not need to be in an acute environment.
These patients are typically elderly patients, who are likely to have one or more chronic condition and sometimes struggle to manage in their own homes.
However, they often require some more support and a period of convalescence, rather than a period in acute care.
Carehome Selection is experienced in designing and operating admission avoidance schemes.
The schemes involve identifying appropriate patients in A&E, identifying appropriate non-acute care for them and monitoring that care, to ensure it is high quality and meets the patient’s needs.
