Hospital co-ordinators provide weekly reports on the status of patients awaiting discharge, enabling planning and placement to take place far more efficiently.

Carehome Selection advisers work directly with the patient and their relatives to identify suitable homes.

Carehome Selection carries out regular care home bed availability checks and hold extensive portfolios on more than 2,000 homes across the country.
