Carehome Selection has been awarded the Health and Well Being contract by Liverpool PCT in a further expansion of services for the city

Carehome Selection has been awarded an innovative Health and Well Being contact by Liverpool Primary Care Trust.

The award is a further expansion of the partnership between Carehome Selection and Liverpool PCT, established to improve patient’s experience of continuing healthcare.

Carehome Selection was first commissioned to carry out a data cleanse of all the information held on 1,500 continuing healthcare patients.

Led by a skilled and experienced manager from Carehome Selection, local staff were recruited to check all the details held on continuing healthcare patients.

Liverpool PCT then purchased Caretrack, Carehome Selection’s tailor-made, web-based database for all continuing healthcare information.

Carehome Selection has now been awarded a Health and Well Being contract, an innovative scheme designed to improve patient experience and reduced delayed discharge.

The scheme is for hospital patients who will need continuing healthcare but no longer require acute medical support. These patients have traditionally faced delays awaiting assessments for their continuing healthcare needs, which is often distressing for the patients and results in an inappropriate use of an acute hospital bed.

All Liverpool patients in this category are now referred to Carehome Selection, who will find them an appropriate interim step-down placement. Carehome Selection manages their care for typically six to 12 weeks after leaving hospital, ensuring the placement is right for the patient while full assessments for continuing healthcare are made.

The Health and Well Being scheme is a major one, involving approximately 40 patients each month.

Kate Craddock, Commissioning Lead for Older People's Services, Liverpool Primary Care Trust describes why Carehome Selection was originally commissioned in the city: "We wanted to bring together all the information which was stored in different parts of the PCT in relation to the funding of people with complex needs.

"We recognised that we needed to amalgamate all this information in order to be able to predict demand for services in the future. "We went to Carehome Selection basis of a recommendation from another PCT.

"The attraction of Carehome Selection was in knowing the company already work in the continuing healthcare sector and understand the relevant policies and initiatives.

"It was not just a case of an IT company asking us what we wanted from a new database.

"It has been very much a partnership – Carehome Selection has advised us on all the different fields and functions we need on the database in order to meet our objectives."
