Plymouth PCT joins Plymouth Hospitals in commissioning Carehome Selection to provide specialist services improving care placements for the elderly

Carehome Selection is working with Plymouth Hospitals NHS Trust to reduce delayed hospital discharges by identifying suitable placements within the community.

Now Plymouth PCT has purchased Caretrack from Carehome Selection, creating a unique partnership with both the acute and primary care trusts.

Caretrack is a unique, tailor-made database designed to manage information about care placements and harmonise financial and administrative information.

Carehome Selection is also carrying out a data cleanse for Plymouth PCT, checking, verifying and entering new information, managing invoices, contracts and commissioning services.

Operations Manager for Carehome Selection, Susan Adams explains: "We are really pleased to be working with both the acute trust and commissioners.

"This provides an excellent opportunity to streamline care from the hospital to the community, offering better services for patients and reducing costs."
